Where one of our group regularily plays a necromancer, and the rest of us enjoy traps, and/or unsuspecting events, this conversation has arisen very often.
In fact I think we had one night where every other 'break' in the game (ex. pizza arrival, someone went to the washroom, etc.) was spent arguing over what/who was truly dead.

Seems to me that I've had or, at least, heard this very conversation more than once in my gaming career, actually... Possibly even right here at the Inn. *wink*
The group I play in usually doesn't ask those kind of questions. If they want to be sure, they just hack it into bits.

Nope...
...sometimes dead lets out a gods-awful howl and charges at you full-bore from an alleyway... then some knucklehead in your party throws a molotov at the thing and nearly cooks you along with the screamer!
Dead turns out to be the last step that guy needed before turning into a horrible undead monstrous creature that proceeds to eat your faces... just saying.
You know, I've never had this discussion with my own players, though they do get nervy when we're still rolling for initiative when everything looks dead!
